Primary care services are co-located with the town MPS and link seamlessly through a single-employer model with the acute services in these towns.
The 4T’s model is one of five innovative pilot projects supported through the Collaborative Care Program to address primary health care challenges in rural NSW. The project is supported by partners NSW Rural Doctors Network and Western NSW Primary Health Network, Local Councils and community groups, and has received funding from the Australian Government.
