Join us as a Rural Generalist leader providing expert primary care services which are patient-centred, address health need and promote wellness across all 4T’s communities within an innovative single-employer model for small rural communities. With Tottenham and Tullamore as the primary base, this role will join senior colleagues already working with the 4T’s network. The Rural Generalist (4T’s) will also support emergency and inpatient services as well as Residential Aged Care in Multipurpose Services across the 4Ts communities.

The 4T’s communities of Tottenham, Tullamore, Trundle and Trangie are all small neighbouring towns in Central Western NSW each with populations of less than 1500 people. Each community is situated approx. 30 minutes from the next by car, and in close proximity to the rural referral centres of Dubbo and Orange.

Tottenham (Pop 453 in 2016) is a recognised as the closest township to the geographical centre of NSW. It is home to a very close knit community, known for their volunteering power and support of one another. Initially a mining town, Tottenham now specialises in the production of wheat and other grains, alongside the production of prime lamb, wool and beef.

Located in central NSW, Tullamore (pop 424 in 2016), has a big mining history. Gold mining and took place in the nearby Fifield area followed by Magnesite later mined for many years. Today, Agriculture is the main industry for the Tullamore area. Tullamore is home to the Tullamore Irish Festival, which is held each Easter long weekend and is the perfect destination if you want to slow down and enjoy the simple things in life.

Since 2019 Western NSW Local Health District has been running a comprehensive primary care service across 4Ts communities with Rural Generalists working in a collaboration team to provide high-quality health care for the region. Primary care services are co-located with the town MPS and link seamlessly through a single-employer model with the acute services in these towns.

The 4T’s model is one of five innovative pilot projects supported through the Collaborative Care Program to address primary health care challenges in rural NSW. The project is supported by partners NSW Rural Doctors Network and Western NSW Primary Health Network, Local Councils and community groups, and has received funding from the Australian Government.

Health professionals embedded into the network receive support from a dedicated team of administrative, nursing and practice staff coordinated by the 4T’s Medical Centre Manager, as well as peer support from across the 4T’s Network.
