HammondCare is forming an exciting new internal Allied Health Care team to work in Residential Care. Whilst having the autonomy to work with residents to achieve the best care outcomes, you will also be part of a broader team of in-house Allied Health professionals working alongside and supporting frontline teams.
Together you will deliver resident-centred, evidence-based care in an environment that reduces excess disability, promotes meaningful engagement in life and strives to implement continuous quality improvement consistent with the values and ethics of HammondCare and the Australian Occupational Therapy Association.
Essence of the role
- Provision of effective and timely occupational therapy to residents and the appropriate support and advice to their representatives and the HammondCare team, at a service level and across residential care, to meet individual needs and preferences.
- Collaborate closely with residents, their representatives, staff, and the broader Allied Health Team to assess functional care needs, set care goals, and provide treatment and interventions to deliver the best possible outcomes for residents.
- Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment.
- Participate in quality improvement projects and continuing professional development.
About You
- Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
- A tertiary qualification in Occupational Therapy and registration as an Occupational Therapist with AHPRA.
- Practical experience or sound knowledge in Aged Care.
- A current driver’s license, safe driving record and access to your own car, with a willingness to travel between HammondCare’s sites and services.
- Effective communication and interpersonal skills (written and verbal)
- Demonstrated organisation and time management skills.
- Demonstrated ability to work independently and as part of an MDT.
