About the Position
NHS has the opportunity for a Community Care Project Officer to work under the direction of the Community Care Coordinator to deliver culturally appropriate services to the aged and disabled members of the Ngaanyatjarra Lands.
The successful candidate will be required to:
- Encourage employment of Indigenous Local people under the NG Lands and Community employment guidelines.
- Supervise and assist community care workers to ensure clients are getting the care they need as directed in their individual support plans.
- Oversee the local meal service and support as required deliver of meals and medication supervision as necessary and in line with the client support plans and direction from Health professionals.
- Oversee and assist clients with social activities at the care centre
- Report to Community Care Coordinator any changes observed in the client or their surroundings that may adversely impact on the client or service delivery and any additional needs identified.
- Maintain all client records and service delivery records on a daily basis, maintain client confidentiality at all times.
- Provide and maintain records of services delivered through the systems provided.
- Ensure maintenance of safe and hygienic working and care environment.
The successful applicant will possess:
- An understanding and appreciation of issues for the Community Care client group within remote Communities.
- Experience in direct service delivery work with the Community Care client group.
- Demonstrated commitment to work with frail aged and young people with disabilities.
- A strong motivation and commitment to the principles of Home and Community Care.
- National Police Clearance or the ability to obtain one.
- Western Australian Open Drivers License and ability to travel via 4WD on unsealed roads.
