Reporting to the Coordinator - Northern Outreach, you will help increase the quality of life and social inclusion of our clients. The service is delivered via an assertive outreach model and achieved by building a purposeful, therapeutic relationship and providing support in a practical and comprehensive way, including a recovery-based case plan.
You will provide direct & practical support and assist clients to access the specialist resources they need such as safe, affordable, and sustainable housing. This is done in collaboration with other health and welfare services as required.
