Reporting to the Coordinator - Northern Outreach, you will help increase the quality of life and social inclusion of our clients. The service is delivered via an assertive outreach model and achieved by building a purposeful, therapeutic relationship and providing support in a practical and comprehensive way, including a recovery-based case plan.

You will provide direct & practical support and assist clients to access the specialist resources they need such as safe, affordable, and sustainable housing. This is done in collaboration with other health and welfare services as required.

  • Work with clients in their homes, on the street, in rooming houses or other locations in the community to make appropriate referrals to external agencies
  • Work with clients regarding basic needs such as emergency accommodation, housing, income entitlements, employment, food, clothing, home support services and medical care
  • Advocate on behalf of clients with other agencies to ensure access and delivery of services, this may require staff accompanying clients to their appointments
  • Work within a collaborative, strengths-based model with a strong client-focused perspective in the development of case plans
  • Work within case planning and monitoring procedures to ensure best results are achieved for each client
