Intereach is seeking a dedicated individual with highly developed communication skills to join the Aged Care Services team in the role of Care Coordinator - Home Maintenance and Modifications.

This position will be responsible for coordinating and facilitating in home services and supports to older people who are eligible to receive Home Care Packages. Care Coordinators will work as part of a team to promote independence and community participation for older people to help them remain in their own homes.
